Social Worker (Part Time, 12-Month Contract) | South Bunbury For Purpose Aged Care, a not-for-profit provider, is seeking a compassionate and experienced Social Worker to join our brand-new South Bunbury residential aged care team on a part-time, 12-month contract. In this rewarding role, you will provide person-centred psychosocial support, advocacy, and case management to residents and their families, helping them navigate important life transitions with dignity, respect, and confidence. A key focus will be supporting residents transitioning from hospital into short-term residential aged care. At Purpose Aged Care, we live our values every day: Collaboration | Accountability | Respect | Empathy About the Role - You will work closely with residents, families, the multidisciplinary team, and external stakeholders to: - Conduct psychosocial assessments and identify support needs. - Provide case management and transition planning. - Support residents and families to understand aged care and living options. - Coordinate referrals to health, community, and government services. - Assist residents experiencing grief, loss, social isolation, or adjustment challenges. - Facilitate family meetings and care planning discussions. - Advocate for resident rights, dignity, choice, and wellbeing. - Contribute to multidisciplinary care planning and quality improvement initiatives. About You
To be successful, you will have: - Bachelor of Social Work or equivalent qualification. - Eligibility for membership with the Australian Association of Social Workers (AASW). - Experience within health, aged care, community, disability, or related services. - Strong communication, counselling, advocacy, and relationship-building skills. - Knowledge of aged care legislation and quality standards. - Ability to work collaboratively within a multidisciplinary team. - Current National Police Check (within 12 months). Desirable:
- Residential aged care experience. - Knowledge of dementia and palliative care. - Current Driver's Licence. Why Join Us?
